You might desire to obtain the book by Dr. Michael Marsh "Coeliac Disease"
It was published in 1992.  It is available from Blackwell Scientific
Publications Cambridge, MA (800) 759-6102 (617) 876-7000.  In 1994, I checked
with the local book stores and they could also order it.  The price started at
$169.00 and my guess is that it is now around $200.00

It is an excellent reference and I recommend it for fellow celiacs who need
information for their physical.
