Actually the Gmail team has made great strides in the last couple of years at making the standard client usable by screen readers. There are several single letter hot keys that are enable d for Gmail, and these can be adjusted in the settings area of the web site.)

G followed by I Go to inbox
J Read next conversation 
K read previous conversation 
O open conversation 
U close conversation 
N next message in conversation 
P previous message in conversation 
# delete a conversation 
E archive a conversation 
R Reply to a message 
A reply all to a message.
? Get a list of google keys

It is possible that some of the above will only work after a check box enabling the key is turned on. Also, the first link on the page (hidden to sighted visitors) is a help document aimed at screen reader users.

Most of my use of the Gmail page has been with MacOS, but certainly the next and previous commands appear to use live ARIA regions to speak appropriate information about the current thread or message.

I am on the opposite end of the spectrum and that I prefer using Gmail on the web. It is simple and has worked for me since 2005 with a screen reader. You need to make sure that basic HTML is your default view. You can do this by choosing Basic HTML as soon as you login and then after that to set as default view. After that, everything should work fine. I haven't had any problems checking email with it, like I said. A few tricks and tips are, ask for checkboxes to get to the Messages quickly.  The B, for buttons   to get to delete, archive, etc. once in an email, go to the bottom of the page with control plus end, then use shift H to go to the last heading on the page. The next thing I do is, press and for nonlink text and usually, that takes me to the body of the message.
