Dear List Members:

    A new aging survey has been developed by members of the list. No matter
what
your age, if you have CP, you are invited to participate in this survey. If
you would like to do so, please email me directly ([log in to unmask]) and I
will send you a copy. The survey is approximately six pages long, but the
vast majority of the items can be checked off. Very little typing is required.
