We generally use OmniPage Pro for the Mac to convert materials to
digital format and then use one of any number of programs that make use
of the Mac's built in text-to-speech feature.  (Word, SimpleText (if the
document isn't too long), AppleWorks, ClarisWorks, TexEdit, and an oldie
but goodie:  MacUser Reading Mouse.

All have some basic built in keyboard commands to controlling the speech
and a macro or two can pretty much customize it for any user.
