Listmembers-

I have requested a letter from my doctor verifying that I have CD.  This
request seemed to confuse his office assistant and she didn't quite know
what it should look like.  Does anyone have a copy of a letter from their
doctor that they could e-mail or fax to me?  I think if I give her some idea
of what it should look like, it would facilitate getting it to me.  Thanks
