       Thanks so much for all the feedback. This list is the best. I want to
especially thank Ron Hogan for his help.  His work can be found at

   Although not everyone was familiar with Ron Hogan's articles, several
sited family histories of the disease.  Many of those requested that I make sure
and do this summary.
   One recommended I do a search on pubmed.
Several posts pointed to vitamin deficiency.
