Feature Article: Services for the Deaf - South Carolina, Department of Mental
Health

While everyone is aware of the more normal applications of telemedicine
technology today, the South Carolina, Department of Mental Health(DMH) has spun
a new wrinkle in the applications “fabric.” DMH is utilizing standard desk top
computers with PictureTel video boards to provide a range of behavioral health
services including services to hard of hearing/deaf clients using American Sign
Language. Computers with seventeen inch monitors and speaker phones combined
with 128K on ISDN phone lines accommodate both hearing and non-hearing client
services in seventeen mental health centers and six hospitals - essentially
linking access to all DMH major service sites.

This innovative paradigm shift occurred nearly four years ago when Dr. Jill
Afrin, Psychiatrist to the Deaf Services Division, notified Mr. Ed Spencer,
Division Coordinator, that she was confined to her home due to pregnancy. Thus,
she was no longer available to drive across the state to sites where she could
provide ASL services to the identified three hundred deaf and mentally ill
clients. Mr.Spencer identified and made arrangements for a video technology
application which would continue to provide access between doctor and client. A
unit was installed in her home and several others placed strategically within
the state. The application was so successful that the rest, as they say, is
successful history! Dr. Afrin reports that she sees three times the previous
number of clients in only sixty percent of the time. Savings accrue due to lack
of staff drive time and related travel costs. DMH has now expanded this initial
service to include many other behavioral health services.

Clients in the community may now go to their respective mental health center
where a Telepsychiatry unit is located. There are seventeen centers throughout
the state, so most clients are relatively close to a major service site. Each
center has an assigned Hard of Hearing/Deaf Services Counselor who assists the
client in scheduling and accessing Dr. Afrin or any other Deaf Services staff
member in the system. As of this date, our system is a point to point
connection.

We have thereby been able to offer greater access, more frequently rendered at
less cost and with 98-99 percent satisfaction rate. Each client and staff are
requested to complete a Satisfaction Survey at the end of each Telepsychiatry
session. Survey items include  satisfaction scales, purpose of session, length,
use of American Sign Language, estimated time and dollar savings, location and
a comment section. Even emergency Hard of Hearing/Deaf Services calls are
addressed more quickly and effectively utilizing this method.

Historically, any of our identified three hundred Deaf clients experiencing
crisis had to notify their regional counselor to determine when Dr.Afrin would
next be traveling to their area and when an appointment may be available. The
time to the actual psychiatric appointment varied and ranged from days to a
week or more. Now, the same notification of crisis and need to contact Dr.Afrin
visually may result in a appointment within hours or a day. Finally, having the
equipment readily accessable to the physician increases the speed of the real
response time between notification of need and actual contact with your doctor.
The nature of crisis calls range from clinical depression to intense family
problems to suicidal ideation. Traditional beeper services are still in use as
well.

The team of Ed Spencer, Dr. Afrin, Barry Critchfield, Ph.D. and Roger Williams
have completed a number of Telepsychiatry demonstrations regionally and
nationally. A number of the demonstrations involve the McKinney House, Deaf
Residential Program, where Roger Williams is Director. The program has a deaf
puppy mascot which responds well to ASL. “Chappy” is always the hit of any
demonstration. SC DMH in December, 1998, was selected as one of the nation’s
Top Ten telemedicine programs by Telehealth Magazine.
