We invite all individuals with celiac disease or dermatitis herpetiformis to participate in a survey study.

The goal of this study is to examine the association between celiac disease/dermatitis herpetiformis and autoimmune disorders.  We are interested in your answers even if you have never had an autoimmune disorder.  Completion of the survey should take about 10 minutes.  This survey includes questions about basic demographics and history regarding celiac disease, dermatitis herpetiformis, diet, autoimmune disorders, allergies, and family history.

Taking part in this study is entirely voluntary.  There is no cost to participate and all information will be kept strictly confidential.  You will not benefit directly as a result of taking part in this study, but knowledge may be gained that might benefit others.
