Thank you all for the many, many nice replies.  They were very helpful.
Most people thought that 2nd degree relatives definitely should be tested
although a few disagreed.  Many said that they or one of their children's
only symptom was constipation.  One person said that Dr. Fasano had
informed them that asymptomatic children under 2 years old should not be
tested because their immune systems were not fully developed.
Suggestions made included typing a statement for the doctor to sign that
he refused to do a celiac test, doing a mouth swab gene test
(www.finerhealth.com  for $49) or enterlab's stool test.
