Dear City of Madison,
The Reverend Dr. Martin Luther King, Jr. Humanitarian Award Commission is beginning the process of selecting the 2016 award winners (flyer attached). This award is presented annually to individuals who have
made outstanding and significant contributions to their community in the spirit of sisterhood, brotherhood, and harmony toward making our city and/or county an ideal place in which to live. Two community members will receive the Humanitarian Award; one Emerging
Leader (ages 13-25) and one Humanitarian (ages 26 and over). We hope that through this award we can identify and recognize new leaders and/or efforts by long-term leaders.
Attached you will find a nomination form and information sheet. Both forms may also be downloaded from the City of Madison’s Department of Civil Rights website at
http://www.cityofmadison.com/dcr/commMLK.cfm. Nominations can be submitted by e-mail, fax, mail, or hand-delivery:
