I've even had the clueless person from food service call my room. I explained everything - blah, blah, blah... My forms were stamped with all the allergens too. I talked to the nurses, they put it in my computer file, the works. Well, in the AM they sent "non-dairy" creamer for my cereal figuring that it couldn't have dairy in it bc it was labeled non-dairy. It was in a glass, so I have no idea what was in it, but probably casein. Fortunately, I was in the maturnity ward and my DH would jsut bring me food in. He'd make sandwiches or whatever at home a mile a way and bring them over when he visited.
