Steve,

There are a few options.  Did you check out Survey Monkey?  That platform is mostly accessible.  I take surveys created by it all the time.  You can also use google Forms.  Microsoft Forms is somewhat accessible but I have found quirks.  You probably don't want to use qualtrix.  I've had issues with some of the surveys created there.  I hope this helps.
