I looked at the survey; and though I didn't actually fill it out or pretend
to fill it out, it appeared "accessible" to me.  I'm not saying it was set
up in a standard way, of course, since it obviously wasn't; and certainly,
radio buttons would make a lot more sense and be easier to work with.  But
at least from looking at it, it would appear that a blind person could fill
it out independently.  Keep in mind that I am using JFW 5.0, though,
too.  Maybe you couldn't tell which link was which in question 2 because
JAWS wasn't set to read certain types of links/graphics, etc., etc.
