At the Script Enabled Blog [its tiny URL given below] is a piece about 
Webvisum, a
Firefox extension that automatically solves visual captchas. Well,
it does much more than that. It allows blind web users to fix issues with
web sites in a crowdsourcing manner, for example by facilitating
social tagging of links and images without alt text. There's a good
interview with the developers which explains its functionality and

(I hear that it works with JAWS9 latest, NVDA, and Window-eyes latest 
version.)
