If I  had to guess, the fax is using a document as a standard cover page
which includes your name and number.  Look in the related menu items for
cover page and see if it specifies a file on your hard drive.  Another
possibility is that a macro is run which has the information you seek, when
you send the fax do you do it by a key combination which is in fact the
command to run a WP macro?
