That depends on your definition of screen reader. I can address Linux. I
think the most complete command line/text console screen reader is a project
called "speakup" at
http://www.linux-speakup.org/
This requires a hardware synthesizer, but gives you access early in the boot
process. Pre-built CD images are availavle for some popular Linux
distributions.

Another option is emacspeak for both Unix and Linux at
http://emacspeak.sourceforge.net/
This gives you access to the emacs environment but you do not have access to
the boot process.

An option for the Gnome desktop environment, a visual environment, is
gnopericus at
http://developer.gnome.org/projects/gap/AT/Gnopernicus/
This is still in early development, but is available in some distributions.

If you use telnet or ssh to access a Linux/Unix machine, you can use your
favorite Windows screen reader on your PC, with a terminal type client such
as Putty or TeraTerm to connect.

There are probably other options, also.
