Michael and List,

Have you tried making it a PDF form?  You can convert a Word Document to a
PDF with relative ease.  Then you just need a PDF editing program to insert
the form fields.  Just remember to label the form fields when you insert
them to make it accessible.  I'm not up on accessible PDF editing programs,
so if you need one maybe some one else on the list knows of one.

Would someone please tell me in detail how to set up an online application 
that is accessible?  I have an application which has been uploaded to a web 
site, however, unless you have the same version of MS-Word as the 
application it will just be a download.  The application is otherwise 
inaccessible.  If you wish I will send you the application for your fixing.

Any help in this matter will be greatly appreciated.
