for more details. Basically you have to had purchased a CD between 1995

2000, max of $20 per person regardless of how many CD's you bought.
Unfortunately, to complete the online claim form you must enter numbers
from
a graphical image for "security reasons" which makes it impossible for a
blind person without sighted assistance. So check the site for exact
qualifications.
