Posting here in hope that someone has knowledge of beta blockers and effect
on thyroid.  History of tachycardia, pulse goes up to 114 even after a bowl
of hot soup at dinnertime.  (Endo had told me, hot soup is actually a
vasodilator!)  All becomes very much more pronounced if my steroids are too
low, Addisons Disease, OR if my thyroid levels are off.

 

Saw a Cardiologist, a specialist in electrophysiology, last Monday.  Comes
very highly recommended.

 

He read through a lot of my notes.  He immediately said, You take very good
notes.  Your symptoms and the patterns from Event Monitor are classic for
adrenal insufficiency.

 

(notes contained description of the bending over/standing up episodes and
tachycardia as a result; standing up and sitting down frequently on
Halloween, etc., hot soup episodes, etc.)  

 

Could there be a 'mechanical' connection?  At times, swallowing food, heart
rate immediately shoots up and is felt in the throat.  Does food sometimes
stimulate sinus node/atrium?  

 

He states, YES this can happen.

 

 

 He repeated that my symptoms are very consistent with addison's.  many
times.  He asked me if I had increased salt intake.  I said, potassium but
not salt.  I am already on high salt in my diet.  He mentioned Gatorade, and
I said I drink the G2, with electrolytes but not so much Sugar.  He nodded.
In addition, he suggests that increasing Florinef could help.  He said,
double it.  And I asked, what about 1-1/2 times the dose?  He said that was
fine to try.  And he wants me to split it, not take 1-1/2 in the AM but
rather take the other half tablet in the evening.  Interesting.  

 

Does thyroid hormone enter the picture?  He read my thoughts on this, that
the presence of thyroid hormone can cause my heart to be sensitive to its
own adrenaline.

 

He said, YES, that is exactly right.

 

 

He then asked if I had considered trying a beta blocker.  I said, not
really, because I do not want to have interaction/symptoms and sometimes
b-blockers can make AI worse.  

 

He said, Yes, sometimes they do.  But a very small amount, a tiny bit, can
blunt the tachycardia a bit.  And you will still know to take more steroids,
because you will still have enough symptoms to know to correct for it.
YES, the adrenal insufficiency will break through the beta blocker effect.  

 

So he gave me a prescription for Indural.  He was going to give me a small
amount of Nadolol (sp?) but changed his mind.  This will be PRN, as needed.
Not every day.  

 

I asked if it is protein bound and if it will affect thyroid.  He said, YES
it is protein bound,  I will give you a very small dose.  And only take PRN.
So should not affect thyroid too much, if at all.  

 

First I am raising florinef the extra half tablet, this for about two weeks.
If continuing issues with HR, then add in the indural as needed.

 

 

We Will See.  I bet I will crash hypoT.  This Indural is brand name for
Propranolol.  And another one, atenolol, might affect T3 conversion less, I
think, from some reading I just did?  Not sure.  Trial and error maybe.  

 

But HEY, maybe when in a bit of tachycardia due to adrenal insufficiency,
maybe I really need to have less thyroid hormone in my system for a bit, to
help settle the situation.  And as long as it isn't every day, but only
occasionally, perhaps I wouldn't even feel the effects.

 

This from someone who is very sensitive to meds.  !!
