Several people have suggested form-filling software, and that's the ideal
solution especially with a form that you're going to be filling in
repeatedly. It may also mean spending money which, if you're job hunting,
you might prefer to avoid.
When I ran into a case like this, I used Photoshop to load the scanned
form as an image, and the text tool to add the information required. That
worked quite well.
I also tried creating a web page in MS Word, with the form image as the
background. That didn't work nearly so well -- IE and Word do not align the
background image to the page text in the same way. It might not have
mettered for normal web work, but it was no good as a form-filling solution.
