Peter's suggestion is very close depending on how picky you want to be
with the logic.  What if there is an single X in either G5 or H5?  It
may be that the correct answer should be a blank cell.  If so then you
would need to put a condition on "Plan D" so it only matches all blank
cells in G5:I5.

=IF(AND(G5="x",H5="x",I5="x"),"Plan
A",IF(AND(G5="x",H5="x",I5=""),"Plan
B",IF(AND(G5="",H5="",I5="x"),"Plan C",IF(ISBLANK(G5:I5),"Plan
D",""))))

If you don't want to leave the results blank, so you can tell it is
working for each possible condition, you could put some text in the
last set of double quotes. i.e. ...,"Plan D","No Match"))))  This would
of course be determined by how the question is actually written.
