Another possible cause is found in taskbar properties. Right click on the taskbar, click properties, and on the taskbar tab, make sure the box that says 'keep the taskbar on top of other windows' is checked. If it isn't, the active window will extend to the bottom of the screen, effectively making the start button "disappear".
