Doug,
I have this setting and it stays in effect nearly all the time. Sometimes,
though, it gets hidden by another window which should not be 'on top'. This
other window also appears to be above the task bar (it doesn't obscure it
but it obscures part of the 3D effect).

When this happens, if I press ctrl-esc I get the start menu, taskbar, and
office shortcut bar all on top.

If I can work out what program is messing things up and end it, things
return to normal. One of the culprits is mcafee virusshield, if started
after windows login.
