I had this problem once before, and now I can't remember how I fixed it!

I did some Win98 updates at the Microsoft site.  Now I'm getting an icon
on my desktop that looks like a blank piece of paper with the right top
corner folded over and the Windows logo on it.  If you double click it,
you get the window that wants you to choose a program to open it.  When
this happened before, the icons would just keep appearing, seemingly
every time I logged on.  A few months ago, someone said it had something
to do with the address book, I think......that there was some glitch in
the Microsoft Windows update, but I can't remember what they said to do
to stop it.  Help?
