I've a very different experience. We just did a complete corporate rollout
of SP5 at the desktop and on all our servers (wide variety of hw), without
incident. As always, I recommend you read MS's complete documentation, with
close attention to known hw/swissues (such as the Lotus Notes issues w/SP6)
before you apply, as well as do a limited test on like configurations for
production implementation. But, we've had absolutely no issues.

Subject: Re: [PCSOFT] NT4, SP3 --> NT4, SP5
I am running sp6 on my NT 4.0 server and NT4.0 workstation with IIS and
Exchange 4.0 with no problems. I would not recommend upgrading past sp5 at
this time though. I have a number of clients running NT with sp5 and have
had no prolems.
