Maxi,
I assume you have a scanner. It depends what you want to achieve with the
form. Scanned forms can be a little difficult to fill in depending on the
program you use. Theoretically it can be done in Word but I hear that is
extra hard to do. The output you want to achieve is important too. Rather
than using a complex form scanning program I suggest you use a draw program
that allows you to enter a background image (the scanned form) then use the
text tool to enter the data on top of the form lines. One program that is
easy to use and can become a valuable asset is:

penny) but you can download the program trial for free and use it (good for
15 days). If after using it you decide to not buy it then just take it off
your computer.
