Hi Dave,

Perhaps Quicktime thinks it should use a different audio device. Try this.

Before loading an MP4, when the sound is operating normally, right click 
on the speaker icon on your notifiction icon area and select "Adjust 
Audio Properties".

Click on the "Audio" tab.

See what is set for the "Preferred Device".

Click "Cancel".

Now go and try to play an MP4 in Quicktime and perform the above steps 
and see if it is still using the same device. If not, then Quicktime is 
changing it. Perhaps in the Quicktime settings there is a preference to 
change the default device.
