Depends on what kind of software came with your scanner. Many, like "Paper
Port" have the ability to annotate your scanned document. If you bring up a
full screen image, there may be a annotation tool bar. Look for the text
button on the bar and click. Then just place the "I" cursor or similar
symbol in the box or line on the form and type in your information. Caveat
is that this is a manual placement andyou may have to experiment with
placement of the cursor symbol.  When you complete, just print the annotated
form. Other word of caution, is to make a copy of the blank form in your
scanner software, since most annotations in this type of software save the
imput as soon as you move on to the next insertion point, without asking if
you want to save annotation.
