The description of this virus is found on

it causes intermittent disk access errors.
It is caused by booting from an infected diskette. It will
also copy itself to every diskette you use thereafter.

basically boot from a clean write-protected diskette. Then
from a clean write-protected diskette run scan c: /clean
/all
