Need to add subtitles to video.  Any instructions and advice and where and how much $$ to buy needed equipment/software would be much appreciated.

This needs to be done in several formats.  The following is as it was explained to me.  If you need more info I will do my best to get that.

Using the last master and ending up with 6 captioned masters.

Last Master    =    Windows Media Closed Caption
                     =    Realplayer Closed Caption
                     =    QuickTime Closed Caption
                     =    Subtitles Version (I think 6) Public Display
                     =    Line 21 Encoded Broadcast Closed Caption
                     =    Line 21 Encoded Broadcast Open Caption

Any information or direction you can point me to would be greatly appreciated.  Feel free to share this if you think someone would be willing to pass on any information also.
