I am sorry that though I am unable to identify the program which has left this file in your windows system folder I feel that the need to reformat/reinstall the os was perhaps because you did not delete the registry entries which could have been made by this unidentified program.
I have always found this to be a problem when deleting windows files/uninstalling programs.
