The last two times I had this problem was when

1) pin #1 was mismarked, so that even though I was carefully lining up the red
marking with pin one, I was still doing it wrong and

2) when I had the Com1 serial cable plugged into the Com2 serial port on the
motherboard, in another case of mismarking.

I finally got very frustrated.  So, in the first case, even though it was
ridiculous, I turned the cable around so that pin one was not going to pin one
(and then the mouse worked), and in the second case plugged the serial port
into which the mouse was plugged into the Com2 port (and the mouse worked --
and reported it was on Com1).
