I had the exact same thing happen to me. I was told, by a tech at the local computer store, that the error message "cdr101 file not found,not ready reading on drive D:" simply meant that the computer could not read the CD and that the CDRom drive was probably bad and needed to be replaced. I thought it was awfully coincidental that the CDRom drive would breakdown at the exact same time as I was doing a reformat so I searched the internet extensively for other answers.

I came across a possible virus connection which I didn't think was likely in my case.

To make a long story short I ended up swapping out the CDRom drive with a known good one and my OS loaded just fine. So, in my case it was a coincidental CDRom breakdown at the same time as a reformat!!!

PS. I did first try more than one boot disc in my attempts to fix the problem. And swapped cables.

PPS. You can't reformat with your boot disc because it must not have the reformat command on it so your computer has no idea what a reformat is anymore.
