 I have a USR 0413;
 the power supply says:
Input -120V AC
Output - 9 volt AC 100 mA
Note the "AC" (alternating current) output,
 as opposed to the normal DC output.

On the front it says:
"US Robotics P/N 1.015.1286
You might try www.3com.com, they bought US Robotics.
