Not too many choices. Either the drive itself has gone bad, the connection is loose on one or both ends, or the cable itself is bad.  It is not impossible that the floppy disk controller on your mobo has gone bad, but that is an expensive possibility.  I had your problem and it turned out that the floppy ribbon cable had gone bad. I bought a new one and all was well.
