This is more likely a problem with the connection of the floppy cable to
the devices or the controller card backing out a bit then it is with the
controller itself. The first thing I would do is to reseat everything. The
second thing I would try to clean the floppy and then check it against
another one. The last thing I would suspect is the actual controller.
