Try each memory chip separately, in the first memory slot.

If they appear to be ok, try all possible permutations, i.e., in your case,
put the new 256 MB DIMM in the first slot and the old 128 MB DIMM in the
second slot. Swapping chips worked for me; I have three chips and only one
of the possible permutations was a problem.

If that does not help, get rid of the old 128 MB DIMM and get another new
256 MB DIMM of the same brand, they are quite affordable today.
