I have used the tekram P5T30-A4 baby AT motherboard on several machines.
Tekram boards have good documentation and are not jumperless.  This board is
socket 7, w/ intel 430tx chipset and supports up to P233 w/ MMX.  It has 4
PCI and 3 ISA slots.

I believe these boards also meet your other specifications adequately.

Cost is almost exactly what you specified, and in 20 paks run about $75.00

Both singles and 20 paks are in stock at TechData Corp.
