A single IDE controller can only handle two IDE devices, Master and Slave.
Some Late model 486 boards have two on-board controllers, PCI bus, and can
even handle 72 pin  SIMM memory.  If the board you are using has a separate
controller card in an ISA slot (And No PCI Bus) you might check it to see if
there is another IDE controller on the controller card, if so you can add
another IDE Ribbon cable and add the new drive to the secondary IDE
controller.  If not, I think you may still be able to find a controller card
that does.
Also, some older SoundBlaster sound cards came with a IDE controller to
handle the CD-ROM, you may be able to run the CD with the sound card and
free-up the slave spot on the primary IDE controller for another HDD.
