Often Windows95/98 will assign an IRQ to the card that will not work.   I have
experienced this when it assigns the card IRQ9.   Try setting the card to
IRQ5 DMA1 DMA5     It should work just fine.  (Some of the later cards
were not able to use DMA5 and you may need to set it to IRQ5 DMA1 DMA3).
