I finally got my Promise card working. Thanks to everyone for their
suggestions. I have enclosed my solution:
Problem:

I could never get a normal boot to try any of the tests. I could only boot
in safe mode so I could not install the drivers for the Promise card. I
finally put the Promise card in slot 1 where the modem card was. I put the
modem card in slot 4 where I had the promise card. The computer booted
normally and I installed the Promise card drivers. Everything works great.
