That is a good point, for rational people who feel they have a reason to use
HRT, topical rather than systemic applications of hormones may produce some
of the benefits with less of the risks. My doctor informs his patients of
the pros and cons, encourages them to research it, and then tells them the
options, which include oral medications, creams, and/or an estrogen ring to
be used internally.

 For example, one data point he offers is that if a woman has been through
early menopause for any reason (such as hysterectomy) and begins oral HRT,
it takes three years to realize the heart benefits.  In the first year, she
is at MORE risk for a heart attack than her peers, in the second year, her
risk is the same as her peers, and only in the third year is her risk of
heart attacks lower than unsupplemented women.

Only the oral medications are systemic, so they may offer heart benefits,
but may also sustain or increase risks for other hormone-triggered
conditions.  Topical treatments such as creams or rings are generally for
dryness/pain only, although if used enough, they can often also alleviate
the specific kind of female incontinence caused by lack of hormones.
