I would start a low carb or zero carb, high fat diet and avoid fructose. 
I would engage in mild exercise and slowly work up to whatever your 
fitness objectives are.
When I was satisfied that the plaques were gone, I'd eat a more balanced 
diet, always  avoiding sugar, especially fructose.
