You don't have to be eating grains to be on a non-ketogenic diet.
Also there is the fact that some people, such as myself,
apparently, seem to experience low blood sugar and consequent
"fight-or-flight" syndrome on a ketogenic diet.  I don't really
know if it is dangerous to be subjected to diet-caused stress
hormones on a regular basis, but it might be.  Obviously, plenty
of other people do not have this experience, so no universal
statement can be made.

Neanderthin is not a ketogenic diet unless you go out of your way
to make it so.  There is no particular reason to expect all
people to tolerate ketosis equally well, any more than there is a
reason to expect them all to tolerate lactose equally well.  Some
do and some don't.  The main thing is to determine by observation
whether one is or is not tolerating *any* dietary strategy.
Remember that Stefansson tolerated an all-meat diet very well for
a year, but Andersen experienced a massive hyperlipidemia.  I
don't think there was any way to predict in advance who would
have which outcome.
