I don't know where to start - the vitriol he spews and the shame and 
othering ...  It would be good if some People Who would comment.

" - no-fault is the key word here, but he means no family fault, it 
is definitely the fault of the person so labeled, who is not 
succumbing to the treatment and therefore must be force-fed.

(DJ, New York ad man, is the one who famously advised families to 
overturn their furniture themselves and then call the police and 
blame their loved one so that person could be carted away.  Sylvia)
