Here are the two responses I received on where to find a dining card in Japanese (thanks!):

(1) The Gluten Intolerance Group (GIG) has a paper copy at no charge. They do not have it available electronically. Contact them at:

GIG
15110 10th Ave SW, Suite A
Seattle, WA 98166-1820
Phone: 206-246-6652
Fax: 206-246-6531
Email: [log in to unmask]

If you send them an email, please give a smart subject line, as they get massive junk emails to filter through.

(2) There is a Japanese dining card in my book:
Jax Peters Lowell, author of "Against The Grain, The Slightly Eccentric Guide to Living Well Without Gluten or Wheat " (Henry Holt).
