The purpose of this survey is to investigate the level
of impact a diagnosis of Coeliac disease or gluten
intollerance has on the emotional welbeing of an
individual.  I aim to compare this across the various
age groups and consider how this affects other areas
of your everyday life such as eating out and use of
skin care products etc.  I would like to take this
opportunity to thank you for your help and support so
far and appologise for the length of time it has taken
me to send out this survey.

PREDIAGNOSIS.
1. At what age were you diagnosed?

2. Please list your symptoms up until this time.

3. In what ways would you say this affected your
social life?

4. How did you feel about your symptoms?

5. What effect did this have on you (what you did,
where you went, how you felt about yourself etc.)

ON DIAGNOSIS.
6. How did you respond to the suggestion that you may
have Coeliac disease or a similar condition/ allergy?

7.how were you diagnosed?
Biopsy/ blood tests/ exclusion diet/ etc.

8. How long did the diagnosis take?

9. How did this make you feel?

10. What was your initial response on learning of your
diagnosis?

POST DIAGNOSIS.
11. How easy/ difficult was it for you to obtain
gluten free food after your diagnosis?

12. how did this make you feel?

13. How often were you tempted to ignore the diet and
eat whatever you felt like?

14. What kind of attitude did the people you came in
contact with have towards your condition?

15. How did this make you feel and what effect (if
any) did it have on your acceptance of the condition).

16. Did you know other Coeliacs who were able to help
you out?

17. What effect would you say this had on you?

18. Were you put in touch with a support network for
Coeliacs, if so, how soon after your diagnosis?

19. Since you started your diet to what level have
your symptoms reduced?

20. Would you say that you have become more socially
active as a resutl of this?

21. Has it become easier to obtain food?

22. Did you do anything to encourage shops and
restaraunts to become more coeliac friendly?

23. How do you feel about requesting special food at
restaraunts?

24. If you entered a restaraunt and found that they
knew nothing about the Coeliac diet what would you do?

25. Do you have any little Coeliac related stories
that you would like to share which may help with my
report.  Names will be changed etc. however this is
entirely up to you.
