Some lucky few must be born with short intestines, or have surgery such
that they have little remaining.  (like after cancer or the like).

They get malabsorption syndromes, like celiacs.

There is another, largely misdiagnosed, syndrome which can cause
malabsorption also, including damage at the microvilli similar to celiac.
It is one that is often missed because the things don't always shed cysts
into the excrement to be seen:
Giardia
This one, though, can be treated.  I believe Metronidazole, 250 mg 3x day.
