CELIAC DISEASE
BLOOD SCREENING

Saturday, October 14, 2000
8am - 2pm

University of Chicago Outpatient Facility
Center for Advanced Medicine Building (DCAM)
5758 S. Maryland Ave.
3rd Floor - Room 3F


Goal: This free screening is to determine the prevalence of Celiac Disease
in first degree relatives of Celiac patients. The samples will be processed
at the University of Chicago for a study that they are conducting. (This is
not the same study that the University of Maryland is conducting.) The test
that will be conducted is the Tissue Transglutaminase Test (it will be a
blood draw from the arm). Participants must be a minimum of one (1) year of
age.

Reservations: Reservations are not required. However, the Center would
really appreciate knowing how many people will be coming so that we will
have an appropriate quantity of the necessary supplies on hand. Please call
the CSA/Greater Chicago phone line and leave a message with your name and
the number of participants you will be bringing. (If you signed up at the
Food Fair, please call and leave a message again with how many people you
will be bringing. This will become the official count.) Your help is greatly
appreciated!

Directions:

From Dan Ryan I-90/94, exit at 55th St. (Garfield Blvd.) and go east. Go
about a 1/2 mile east to Washington Park. Follow the blue "H" signs through
park to the U of C Hospitals. At 57th St. turn left. At the corner of
Cottage Grove and 57th St., go straight (east). Go one block to Maryland
Ave. and turn right. Go one block south to the Center for Advanced Medicine.

From Lake Shore Drive (north), exit at 57th St. (at Museum of Science and
Industry). Follow 57th St. to the left as it curves around the museum and
becomes Cornell Dr. Shortly after the museum, turn right onto Midway
Plaisance (just after U of C sign). Follow the Midway Plaisance west through
the University campus to Cottage Grove Ave. Tun right. Go three short blocks
and turn right again at the traffic light at 57th St. Go one block east and
turn right onto Maryland Ave. Go one block south to the Center for Advanced
Medicine.

From the Skyway I-90, exit at Stony Island Ave. Follow Stony Island Ave.
north to the Midway Plaisance (after 60th St.). Turn left onto Midway
Plaisance and continue to Cottage Grove Ave. Turn right. Go three short
blocks and turn right again at the traffic light at 57th St. Go one block
east and turn right onto Maryland Ave. Go one block south to the Center for
Advanced Medicine.

Parking:

There is street parking in the vicinity (i.e Midway Plaisance). There is
also parking on the premises with valet or self park options. Fees range
from $3-7 for less than 6 hours. Parking is also available at the Hospital.
