It seems that many people are using restaurant cards.  I found that
there are free ones online as well as one that are paid for.  Janet
did a very nice job of summarizing restaurant cards for me.

As for the cross contamination issue, I had one person send me a
sample of a card.  A couple of people told me that the Triumph Cards
addressed the issue.  Others seemed to think that since celiac disease
is not life threatening in terms of an anaphalactic reaction, cross
contamination issues weren't addressed as often on restaurant cards.

For our family, I am going to be using cards that address cross
contamination.  While I understand that eating out always puts us at
risk, sometimes going out to eat cannot be helped.  It's especially
the case when traveling.
